Request Slate CRM assistance for: account/profile creation, locked and password resets, communication campaign forms and configurations, and training.


Request ID card access to spaces that use an electronic door locks, access to parking ramp and other student-related services.


Reserve equipment for events in the Great Hall, Founders Hall Auditorium, Founders Hall Reception Area or Ecolab.


Request a new shared drive folder, access to an existing shared drive folder, or removal of access to an existing shared drive folder.


Request new desk phone, reassign an existing desk phone or request configuration changes to your desk phone.


Students and employees can attach photo and submit this request for new ID Card or replacement card.


Request 8 ½ X 11 white copier paper for delivery.


Ability to request a set-up including daily standard, max seating and/or a custom set-up within an event space.
