Physical Key Request

Overview

This form is used to request a physical key that grants access to a key-only space.

  • The first key issued is provided free of charge.
  • Replacement or keys not returned will incur a $30.00 charge to the key holder.

Once request is processed, physical keys can be picked up from the Mailroom Service Center (SJH L3) via the service window - requests are typically processed within 3 business days.

  • If key(s) are not picked up within 30 days of request, key will be put back in inventory and a new request will need to be submitted. 

Upon separation of employment, key holders are responsible for returning all issued keys to your supervisor; the supervisor/dean is responsible for returning assigned keys to Mailroom Service Center (SJH L3) via drop off window or inner office mail.

Replacement costs for re-keying lost or misplaced keys are the responsibility of the individual dept. of the areas affected.

Campus Operations takes pride in the safety and security of the campus therefore an annual audit will be performed to make sure all keys are accounted for.

Eligibility

Employees with dean or supervisor approval are eligible to request this service.

Note: If you are submitting this request on behalf of another individual, please list their name as the requester. The key holder (not the requester) must be present to sign for the key — please plan accordingly.

How are Keys Managed?

  • Every key assigned to an employee is documented and tracked through the service management request process.

How are Master Keys Assigned?

  • Suite master keys are intended for supervisors and office managers only. All keys must be approved by the requesting employee’s supervisor before the key will be issued.
  • Building master keys and exterior master keys are intended for limited individuals. All key requests for these keys must be approved by the requesting employee’s supervisor and approved by the CFO before the key will be issued.
