Student: Legal Name Change

  • Students who have legally changed their name or social security number should notify Gateway Student Services of the change.
  • After your documentation has been submitted and approved, Gateway Services will notify Minnesota State IT Services on your behalf and follow-up with you.


Process:

  • Students and employees must submit a written request using form > Name or SSN Change.
  • Identification is necessary for submission, requiring the requester to submit the form in person or have officially notarized prior to mailing.

Students:

  • Submit the completed Preferred Name Request Form, in person or by mail, along with legal documentation to Gateway Student Services.
