Process:
- Students and employees must submit a written request using form > Name or SSN Change.
- Identification is necessary for submission, requiring the requester to submit the form in person or have officially notarized prior to mailing.
Students:
- Submit the completed Preferred Name Request Form, in person or by mail, along with legal documentation to Gateway Student Services.

- Once your documentation has been submitted and approved, Gateway Services will notify Minnesota State IT Services on your behalf, and follow-up with you.
