Preferred Name Change

  • Preferred Name: A preferred name is a name that a person has chosen to identify themselves, where legally permissible, within the Minnesota State system that is different, in whole or in part, from their legal name.
  • Employees and students can request use of a preferred name.

  • After your documentation has been submitted and approved, Gateway Services (for students) or Human Resources (for employees) will notify Minnesota State IT Services on your behalf and follow-up with you.


Process:

  • Students and employees must submit a written request using the Preferred Name Request form.
  • Identification is necessary for submission, requiring the requester to submit the form in person or have officially notarized prior to mailing.
